故事发生在明朝年间，叶常绿（黄德斌 饰）是朝廷刽子手，因为奉命斩首了恩公吕大人而心怀愧疚。一次偶然之中，叶常绿邂逅了吕大人身怀六甲的小妾阮素心（姚子羚 饰），将其救下，藏在京城第一稳婆花蕊红（邵美琪 饰）家中待产，并威胁后者不准将此事泄露出去。
一天夜里，叶常绿外出寻找失踪的花蕊红，两人却因此救下了一具女尸腹中的胎儿，哪知道这个来路不明的婴儿竟将两人卷入了一场朝廷内部的明争暗斗之中。万贵妃（康华 饰）产子，花蕊红被召入宫中接生，最终胎儿平安诞生，花蕊红因此和万贵妃成为了好友，却也得罪了将万贵妃视为敌对的吴皇后（陈卓盈 饰）。
